Abstract
Renal pathologic changes correlate poorly with the degree of physiologic impairment in patients with acute renal failure (ARF). The evidence against earlier etiologic theories, such as tubular obstruction and passive backflow, is convincing. Recent experiments have demonstrated preferential cortical ischemia in patients with ARF, suggesting altered hemodynamics due to increased vascular resistance. Morphologic changes in the kidney and the clinical renal failure are probably parallel results of the same underlying functional abnormality.
