Funding Information:It is of interest to note that none other than Michael DeBakey, one of the founders of cardiovascular surgery in the United States, proposed to the US Congress as long ago as 1959 the nationwide establishment of specialized medical COE. Although growth of a network of COEs was limited by available funding, such COEs were strongly urged in the report of President Johnson’s Commission on heart disease, cancer, and stroke ( 30 ). University-based programs, devoted largely to research and education, grew rapidly over the following 20 years, funded by the National Institutes of Health and private concerns. Recent developments in our health care systems have since returned the focus to patient outcomes and cost effectiveness, and COEs based upon these goals have been expanded over the past decade ( Figs. 5 and 6 ).
