Abstract
Home care is required to collect increasing amounts of information. The Health Care Financing Administration recently published the new Conditions of Participation requiring the collection and use of the Outcome and Assessment Information Set (OASIS) to demonstrate outcomes-based quality improvement. By itself, the OASIS data set is not a comprehensive assessment. When combined with the Omaha System, agencies can conduct a comprehensive assessment, as well as document interventions and additional outcomes. CareFacts Information System has incorporated the OASIS data set along with the Omaha System to provide a comprehensive computerized information system. Strategies for successful implementation of the Omaha System within CareFacts in one home care agency is discussed along with the lessons learned, benefits of computerization, and future recommendations.
