Abstract
The experience of the International Society of Addiction Medicine in setting up the first international certification of clinical knowledge is reported. The steps followed and the results of a psychometric analysis of the tests from the first 65 candidates are reported. Lessons learned in the first 5 years and challenges for the future are identified.

Funding Information:There is no doubt that language proficiency can be a barrier. Discussions are underway to translate the examination into Spanish and Italian. Settings where a computerized version of the examination can be administered are being contemplated. The security of this process may be improving. The cost remains significant. The search for funding support is ongoing. Several examination sets are being administered by different academic institutions worldwide. Must we reinvent the wheel time and time again?
